The registration form is an Adobe Form.  You will need Adobe Reader to see, fill out, and/or print the form.

You can fill the form out online by simply clicking in the "Name of Group" field and starting to type.  Please use the TAB key to go on to the next field.

When you are finished filling in the form, you can click on the "Send Form" button and Adobe Reader will step you through the process of e-mailing the data.  (If you're viewing the form in another application, you can also save it and e-mail it as an attachment to MovementCenter [at] aol [dot] com.)

You can also click the "Print Form" button to get a copy of the form for your records, fill out the form by hand, or to send via snail mail.

If something goes wrong along the way, either clicking the "Reset Form" button (or the "Reload" button on your browser) will allow you to start over.
